Thank you for taking the time to tell us about your organization. We hope that the work your board puts into this self-assessment tool will be beneficial and informative for everyone! CFN appreciates your efforts and will treat your information with respect.

Prepping your answers
It will be handy to have bylaws and officer information available when you are ready to fill out the form. The form will ask you about:

  • Incorporation date 

  • Schedule and location of meetings (membership and board)

  • Membership numbers, activity, and dues

  • Composition of the board

  • Terms of office and recent election date  

  • Names and contact information of officers

  • Status of the organization (by-laws, incorporation, financials)


You might also wish to think through the following questions as an organization before completing the annual review:

  • Please list the streets / features that define your organizational boundaries to the North, South, East, and West.

  • What are your organization’s top three current priorities/issues/concerns?

  • Please describe any issues/project areas where your organization might require assistance from an outside expert or official.

  • What are your organization’s historic strengths or skill sets?

  • List the projects, events and/or issues your organization addressed in the last year.


Form Completion
Once you’re ready, the online version will take approximately 15-20 minutes to complete. Please be aware that information entered will not be saved until you have clicked the Submit button at the end. After submitting, you can edit your responses as long as the window is open; just remember to re-save! If you need to make changes after submitting and closing windows or if you prefer to complete a hardcopy of the form, please just give us a call, and we’ll be happy to help.
